Because you deserve connection

As a Social Worker with hospital experience, I know how hard your care team works and how far a little communication from mental health professionals can go to help you get what you need.

How can you help me with my Care Team?

  • I define “care team” as any other providers you’re working with such as Medical Professionals, Physical Therapists, Nutritionists/Dietitians, Other Mental Healthcare Professionals etc.

    Or, maybe there are some other providers you’d like me to communicate with, such as an alternative healer or your personal trainer.

    Whoever you see as an aid to healing in your life, that’s your care team.

  • Absolutely not. You get to organize your care in whatever way you see fit. You can decide to allow me to communicate or not communicate wit your providers at any time and this is something we can discuss at any point in your treatment. Initially, I will not contact any providers until you have signed a release form, which is a process we will look at together.

  • Sometimes, offices are interested in receiving a copy of my notes so they can better follow up with you and know a bit about what’s been going on in your life since your last visit.

    Other times, providers will want to get on the phone with me and have discussions about your care. I find these discussions can be a source of inspiration, for both parties involved.

    Regardless of what your and your providers preferences are, know I am invested in your and your care and will do my best to be the support system you and your providers need.

    NOTE: I am only able to work with care teams who respond to my requests and do my very best to return care team calls within 24 hours, Mon-Thurs.

  • Nope! Not at all! I see this part of my work, which I genuinely enjoy, as part of my fee.

  • First of all, it’s totally understandable to be in-between providers the way our systems are currently organized. I might not have the perfect referral for you in your area, but we can definitely make Care Team Creation part of your goals during our sessions - I am in!
